All of our home care services are managed by an RN Case Manager. Case Managers are skilled in evaluating the individual clinical, physical, emotional and social needs of each client. Case Managers undergo significant orientation and training to understand the various reimbursement options that are available in order to provide advice that might lesson the financial burden on clients and/or their family members for paying for our services.
Case Managers assume responsibility for coordination of client records in the home and the office. They also coordinate client cares with the family, other caregivers and community resources. The various types of services we provide include:
